ANNUAL VISITS & WELLNESS EXAMS
Comprehensive, personalized care for adults, with limited pediatric in-home services in Florida
Your annual visit is more than a routine check-in. It is dedicated time to review your health, address concerns, and create a plan that supports your long-term well-being.
At Curing Grace Wellness, annual visits and wellness exams are designed to be unrushed, collaborative, and focused on prevention, early identification of concerns, and continuity of care. We take time to listen, review your history, and partner with you in informed, thoughtful care.
Adult annual visits and wellness exams are provided via secure telehealth in Florida, Iowa, Virginia, Arizona, Colorado, Connecticut, and Maine.
In-home annual visits for children and teens (ages 6 and up) are available in Florida only, including Milton and surrounding areas.
What’s Included in an Annual Visit or Wellness Exam
Preventive & Ongoing Care
Comprehensive health reviews, age-appropriate screenings, medication management, and personalized preventive care planning.
Chronic & Lifestyle-Related Health Review
During annual visits, we identify, review, and discuss chronic or lifestyle-related health concerns that may impact your overall wellness. This visit often serves as a starting point to recognize patterns, review prior diagnoses, and determine appropriate next steps.
Concerns commonly reviewed during annual visits may include:
Thyroid conditions, including Hashimoto’s and hypothyroidism
PCOS and hormone-related concerns
Weight or metabolic health changes
Fatigue, sleep concerns, or brain fog
Blood pressure, cholesterol, or blood sugar concerns
When ongoing or more focused support is needed, care may continue through follow-up visits or condition-specific services.
Acute Concerns Addressed When Appropriate
Minor or time-sensitive concerns that arise during your visit may be addressed when clinically appropriate and within scope.
Medicare Annual Wellness Visits (AWV)
Medicare Annual Wellness Visits focus on preventive care, health risk assessment, and personalized prevention planning. These visits do not include a hands-on physical exam or management of new or acute medical concerns.
When appropriate, concerns identified during an AWV may be addressed through a separate visit or follow-up appointment.
Eligibility and coverage are based on Medicare guidelines.
Pediatric & Teen Wellness Visits (Florida In-Home Only, Ages 6+)
For families located in Florida, in-home wellness visits for children and teens may include:
General health and developmental review
Preventive guidance and parent education
School or sports physicals when appropriate
Routine pediatric preventive care with a local in-person provider is encouraged.
What to Expect From Your Visit

Phase 3: Ongoing Support
Follow-up visits may be recommended to support continuity of care or to address specific concerns in more depth.
